A hollow is a soul that's never been crossed over. Usually, after being out for so long, they snap and go crazy and try to kill the ones they love the most, and then they just start to eat souls at random to grow in power and strength. If they're killed by a zanpakutou, they're purified. Hollows always have masks, but the less prominent it is, the stronger they are. Like, some of them have a mask covering their entire body, while others just have it so they look like glasses or something.
